Referrals and Chronic Conditions

BMee works with a number of referral partners to help manage chronic conditions.

Our Exercise Physiologists are the exercise specialists. Prescribing you safe, evidence-based exercise interventions which are individualised and specific to you and your injury, condition or chronic disease.

Our Dietitians are specialists in the area of food and can effectively manage medical conditions, navigate IBS or food intolerances, provide guidance for weight management, diabetes management, sustaining good gut health, and nutrition advice for mental health.

Patients with chronic health conditions may be eligible for referral to an Allied Health professional under Medicare’s Chronic Disease Management Program scheme. These referrals can cover Exercise Physiology, Dietitian and Diabetes Educator.

An Chronic Disease Management referral entitles the patient to a maximum of 5 individual sessions with an Allied Health provider in each calendar year.

Depending on their condition, patients can be referred to multiple Allied Health providers to achieve the best team care approach for their health. Please speak to your General Practitioner to discuss your eligibility for an Chronic Disease Management referral.

Under the mental health scheme you can be referred up to 20 visits per year for eating disorders.
